2012 Awards Packet
Definitions:
The term current year refers to the time period from January 1 to December 31, 2011.
The term good standing means that an individual or chapter has paid all chapter, regional and/or international dues.
To be eligible for an individual award, a nominee, author, or artist, must be a member, in good standing, of a Phi Theta Kappa chapter. To be eligible for a chapter award, the chapter must be in good standing.
Each nominee must submit an appropriate entry form with original signatures (no photocopies of signatures) of both the advisor and chapter president, and individual applicant, if necessary.
Changes to Regional Awards:
• The Yearbook criteria has been revised to reflect the College Project and Honors in Action Project
Entry forms for all Regional awards, with the exception of the Yearbook, Art, Travel/Participation Awards, must be submitted directly to the Nevada/California Regional Coordinator by the entry deadline.

Description of Award: A plaque and $ 50 stipend for the most outstanding art entry: the entry for the Art Award may be in one of two categories:
I STATIC • an original painting or sculpture in any medium; or a still photograph (must be on display by the start of the Saturday Morning General Session. No exceptions!)
II FLUID • an original musical composition or video. ENTRY CANNOT BE MORE THAN 3 MINUTES IN LENGTH.
It is permissible for one person to have one entry in each of the two categories. (Two entries by one individual in a category will disqualify both entries.)
Consideration will be given to originality, presentation, creativity, technique, skill with the medium, clarity of idea, neatness, and overall effect.
Entry Specifications: Each applicant (there may be several from the same chapter) may submit, in each of the arts categories described above, if desired, only one original work to be considered for this award. The piece must not have won recognition at any previous Phi Theta Kappa convention.
This form (or a copy), with original signatures, and the artwork must be brought to the Regional Convention for display by the start of the first General Session. The FLUID art entry must be submitted by mail or electronically and received by the Regional Coordinator by the February deadline.
It is the responsibility of the entrant to ensure that the work is properly delivered to the Awards Chairperson by the start of the Regional Convention. The host chapter assumes no responsibility nor liability for the loss of or damage to the work; however all reasonable efforts should be made by the host chapter to protect it from damage or loss.

Description of Award: A plaque and $ 50 stipend for the best presentation of a chapter yearbook. The entry for the Yearbook Award should be a “portfolio” of the chapter’s activities for the calendar year Jan. 1 - Dec. 31, 2011. Judging will be based on appearance, content, theme, and presentation of chapter accomplishments as specified in the Yearbook Judging Criteria.
Entry Specifications: Submit the Yearbook , three ( 3 ) copies of this Entry Form , and one copy of the Judging Criteria (next two pages) with original signatures; must be brought to the Regional Convention for display. Yearbooks must be on display by the start of the Saturday Morning General Session. (No exceptions!)

2011 Yearbook Award Checklist / Judging Criteria
1. Cover ( 5 points )
a. Does it establish the overall yearbook theme ?
b. Does it create sufficient viewer interest to inspire exploration of its contents ?
c. Does it include the name of the chapter, name and location of the college, and the year?
2. Overall Theme ( 5 points )
a. Does the yearbook have an overall, unifying theme ?
b. Is the theme carried out in such a way that it adds interest rather than distracts ?
d. Is there continuity of presentation throughout the yearbook materials, photos, lettering, etc. ? Does the yearbook hold together as a complete unit rather than an accumulation of isolated pages and ideas ?
3. Divisions ( 5 points )
Does the yearbook include the following divisions:
a. Honors Study Topic
b. College Project
c. Honors in Action Project

f. Are there newspaper stories or personal accounts included to indicate PHI THETA KAPPA members' involvement in other campus and community activities?

The Participation Award is a plaque and $ 100 stipend presented to the chapter that brings the highest total number of members and advisors to the five listed conferences and conventions held during the current year.
The Travel Award is a plaque and $ 100 stipend presented to the chapter that travels the greatest accumulated one–way distance (as computed by Mapquest from college campus to destination location) to the various listed conferences and conventions held during the current year. A host chapter is given one mile for the conference that it hosts in computing its accumulated distance for the year.
THIS FORM MUST BE SUBMITTED TO THE AWARDS CHAIRPERSON NO LATER THAN
THE START OF THE SATURDAY MORNING GENERAL SESSION OF THE REGIONAL CONVENTION.
In case of any discrepancy, Regional or International records will prevail.

Description of Award: A check for one hundred fifty dollars ( $ 150.00 ) awarded to a Regional Phi Theta Kappan who has an academically outstanding, well–rounded, excellent, community college record.
The check will be presented to the recipient, or the next runner up, during the fall term of the next academic year, after the recipient, or the next runner up, provides proof of enrollment at a four–year college or university.
Entry Specifications and Criteria for Consideration :
1. The applicant must be a member in good standing of a regional chapter that is in good standing ( must be verified by chapter advisor ).
2. Considering only the last five years, the applicant must never have dropped any course during one's junior/community college academic career (i.e., official transcripts should not contain any W's) .
3. The applicant should have taken a broad spectrum of courses from a variety of divisions, departments, disciplines, etc.
4. The applicant should have a high overall cumulative grade point average.
5. The applicant must submit official transcripts by the entry deadline verifying items # 2 – 4 above, from all two year colleges attended within the last five years.
6. The applicant should be in the process of completing one's community college course work during the current year, and must transfer to a four year college/university in the upcoming fall term; failure to transfer will void the award.
More than one applicant from the same chapter may apply for this award.

We have verified through our College Registrar/Admissions & Records Office that the above applicant has provided all transcripts from all community colleges attended within the last five years, and that the applicant is in good standing with our chapter, and that our chapter is in good standing in the Nevada/California Region.

Description of Award: A plaque and $ 75 stipend presented to a newly chartered or reactivated chapter, in good standing, that has had the greatest development or renewal of vitality and dedication during the current year. A newly chartered chapter is one that would count the current year's regional convention as the conclusion of, at most, two full years of existence. The beginning of the first full year is marked by the first Regional Convention for which the chapter is eligible to attend (regardless of whether the chapter actually did attend) after it's chartering date. A reactivated chapter is one that has been active for at most the last two ( 2 ) years, and inactive for the preceding four ( 4 ) or more consecutive years.
Entry Specifications: Materials in all three Hallmarks (as submitted for International Hallmarks Awards), and an original essay of no more than (approximately) 1000 words or four ( 4 ) typed pages double-spaced, describing the progress the chapter has made during the current year. Compare and contrast it with that of the previous year. A mere list of activities or a mere count of how many members were present at each event may be necessary but not sufficient.
It is recommended that the essay include a sense of involvement, enthusiasm, and accomplishment that the chapter experienced in carrying out the goals of Phi Theta Kappa, and how that success related to the 5–Star Program.
Additional (optional) material that can be submitted might include:
• a calendar describing the chapter's activities during each of the previous and current years
• articles from either the college or local newspaper describing the chapter's activities
• a letter from an advisor, officer, and/or member who was active the previous year
• a letter from either an officer or member who was active in both the previous and current years (to give a sense of contrast, growth, continuity, etc.)

Description of Award: A plaque and $ 50 stipend presented to a member (non–officer) of the chapter who has been judged most outstanding within the Region.
2011 Outstanding Chapter Officer Entry Form
Description of Award: A plaque and $ 50 stipend presented to an officer (other than the president) of the Chapter who has been judged most outstanding within the Region.
2011 Outstanding Chapter President Entry Form
Description of Award: A plaque and $ 50 stipend presented to the President of the Chapter who has been judged most outstanding within the Region.
2011 Outstanding Chapter Advisor Entry Form
Description of Award: A plaque presented to the Chapter Advisor who has been judged most outstanding within the Region. A credit stipend of a complimentary convention registration is presented to the advisor to be used at one of the remaining year's NV/CA Regional conferences (2012 Summer Honors Institute, 2012 Fall Leadership Conference).

Honors In Action
Description of Award: A plaque and a $ 50 stipend presented to the highest ranking chapter (in the NV/CA Region) in each of the “Ten Issues” categories of the HIA Program.
College Project
Description of Award: A plaque and a $ 50 stipend presented to the highest ranking chapter (in the NV/CA Region) in this category.
The Outstanding Chapter Hallmark Award requires (at least) two of the four following activities:
In addition to the standard requirements for this chapter award, chapters applying for this award MUST ALSO have participated in at least two ( 2 ) of the Regional Projects:
You must report your involvement and activities to the appropriate Regional Officer by Dec. 31, 2011.
i) Make-a-Difference Day: a community service activity held on the fourth Saturday of October of this year (Oct. 22, 2011)
